This thesis is a reflection of a research project performed in the Emma Children’s Hospital, Academic Medical Center, University of Amsterdam in the period 1993-2008. Apart from the work described in Addendum 3, all research was performed within the scope of two Dutch national projects. Firstly, the TBG project, initiated by the Department of Pediatric Endocrinology of the Emma Children’s Hospital AMC and coordinated by the National Institute of Public Health and the Environment (RIVM), Bilthoven and TNO Prevention and Health, Department of Social Pediatrics and Child and Youth Health Care, Leiden, in collaboration with the National Advisory Board on Congenital Hypothyroidism. Aim of the TBG (Thyroxine-Binding Globulin) project was to improve the procedure of the neonatal mass screening for congenital hypothyroidism (CH), especially regarding the false-positive rate and regarding detection of patients with congenital hypothyroidism of central origin. Secondly, the CTDS study, designed, coordinated and performed by Thomas Vulsma, Jan de Vijlder and the author of this thesis (from the Emma Children’s Hospital AMC), in collaboration with the National Working Group on Growth Hormone Research and the endocrinologic reference laboratories from the Academic Medical Center, University of Amsterdam, Free University Medical Center, Amsterdam and Wilhelmina Children’s Hospital, University of Utrecht Medical Center. Aim of the CTDS (Congenital Thyrotropin Deficiency Syndromes) study was the development of a diagnostic strategy after an infant was screened positive for thyrotropin (TSH) deficiency. The strategy consisted of tests of hypothalamic-pituitary function and magnetic resonance imaging (MRI) of the hypothalamus-pituitary region and other areas of the brain.
